When calling to schedule your first appointment, please have ready:

• Your physician's name and office phone number
• The part of your body for which you are seeking treatment
• Your insurance information

To assure the best care for you, we ask that you bring the following with you to your first appointment:

• Insurance Card
• Prescription from your physician, if applicable
• Appropriate clothing:
  • Knee injury: shorts, t-shirt, sneakers
  • Shoulder/elbow injury: short sleeve/sleeveless shirt
  • Neck/Back injury: comfortable clothing such as sweats, shorts, etc.
  • Ankle/Foot injury: shorts or slacks so the pant leg may be rolled up, sneakers

The first appointment includes the initial evaluation and the first treatment. This visit is approximately 1 hour and 15 minutes to 1 hour and 30 minutes. Subsequent visits are approximately 1 hour.
